How to Apply

ANM (Auxiliary Nurse and Midwifery Training Course) Application For Admission

Application must be made on prescribed form to The Principal Aaditya Group of Institution. It must be accompanied by the following documents :-

01An attested copy of certificate of metric.
02An attested copy of detail marks card of metric.
03An attested copy of certificates / detail marks card of 10+2 pass.
04Six recent passport size photographs, one of which must be pasted in the space provided
05Character certificate from the principal of the school/college last attended.
06A duly sworn affidavit as formatted on non Judicial stamp paper of Rs. 25/-.
07A Certificate as formatted from the sarpanch/Councillor concerned.
08A bond as prescribed with Counter Signature and address of the father or mother or guardian. However, in case of minority of a candidate the bond should be executed by his father or mother or guardian. This bond is to be furnished only after the candidate is admitted to course.
09An application complete in all respects must be submitted either through regd. post or personally and should reach the office upto 3 PM. of the last date to submit the applications, Incomplete applications or those received late shall be rejected summarily. Once an application is submitted, no change will be allowed thereafter.

 

GNM Nursing (General Nursing and Midwifery) Application For Admission

Application must be made on prescribed form to The Principal Aaditya Group of Institution. It must be accompanied by the following documents :-

01An attested copy of certificate of metric.
02An attested copy of detail marks card of metric.
03An attested copy of certificates / detail marks card of 10+2 pass.
04Six recent passport size photographs, one of which must be pasted in the space provided
05Character certificate from the principal of the school/college last attended.
06A duly sworn affidavit as formatted on non Judicial stamp paper of Rs. 25/-.
07A Certificate as formatted from the sarpanch/Councillor concerned.

B.Sc. Nursing 4 years Degree Course Application For Admission

03An attested copy of certificates / detail marks card of 10+2 pass with medical subjects.
04An attested copy of SC/ST category proof.
05Six recent passport size photographs, one of which must be pasted in the space provided
06Character Certificate from the principal of the college last attended.
07A duly swron affidavit as formatted on non judicial stamp paper of R
s.25/-
08A certificate as formatted from the sarpanch/councillor concerned.
09A bond as prescribed with counter signature and Address of the father or mother or guardin. This bond is to be furnished only after the candidate is admitted to course.

Post Basic B.Sc. Nursing – 2 years Degree Course Application For Admission

Application must be made on prescribed form to The Principal, College of Nursing Sigma Nursing Training Institute, Ludhiana. It must be accompanied by the following documents:-

01An attested copy of certificate of matric.
02An attested copy of detail marks card of matric.
03An attested copy of detail marks card of 10+2 pass.
04An attested copy of each of the certificates/Detail Marks Cards regarding the passing and marks obtained in G.N.M.
05An attested copy of registration certificate with RN Number & RM Number issued by State Nursing Registration Council.
06An attested copy of SC/ST Category Proof.
07Six recent passport size photographs, one of which must be pasted in the space provided.
08Charater Certificate from the principal of the college last attended.
09A duly sworm affidavit as formatted on non judicial stamp paper of Rs.25/-.
10A certificate as formatted from the Sarpanch/Councillor concerned.
11A bond as prescribed with Counter Signature and Address of the father or mother or guardian. This bond is to be furnished only after the candidate is admitted to course.

M.Sc. Nursing (OBG, Psy, CHN) Application For Admission

 

M.Sc. Nursing (OBG, Psy, CHN) Application For Admission

Application must be made on prescribed form to The Principal, College of Nursing, Sigma Nursing Training Institute. Ludhiana. It must be accompanied by the following documents :-

01An attested copy of certificate of matric .
02An attested copy of detail marks card of matric.
03An attested copy of detail marks card of 10+2 pass.
04An attested copy of registration certificate with RN Number & RM Number issued by State Nursing Registration Council.
05An attested copy of each of the Certificates/Detail Marks Cards regarding the passing and marks obtained in B.Sc. (Nursing)/B.Sc. Hons. (Nursing)/Post Basic B.Sc. (Nursing).
06An attested copy of Work Experience after Basic B.Sc. (Nursing).
07An attested copy of prior or after work experience in case of Post Basic B.Sc. (Nursing).
08An attested copy of SC/ST Category Proof.
09Six recent passport size photographs, one of which must be pasted in the space provided.
10Character Certificate from the principal of the College last attended.
11A duly sworn affidavit as formatted on non judicial stamp paper of Rs.25/-.
